MICHIGAN'S BUREAU OF HEALTH SYSTEMS: HOW TO FILE A COMPLAINT AGAINST A NURSING HOME, HOSPITAL, AND OTHER HEALTH CARE FACILITIES

Family members of nursing home residents, hospital patients, and other health care clients, with an immediate or urgent concern with a Michigan based facility can file a complaint with the State of Michigan Bureau of Health Systems (BHS) via its on-line complaint form.
The first step in filing a complaint with BHS is to follow the chain of command by contacting the facility directly and voicing your concerns with the person(s) involved. If you’re unable to reach a resolution, follow-up with a written complaint addressed to their supervisor and/or the director of the facility. By law, all nursing homes and health care facilities are required to post contact information for the onsite personnel responsible for receiving complaints and conducting investigations. This information should be readily available at all times. Be sure to keep an accurate account of your concerns including relevant dates, times and documentation.
The second step is to file a complaint with the BHS via their website at www.michigan.gov/bhs and follow the link for "BHS Health Facility On-line Complaint Form”. For individuals wishing to mail or fax their complaint, they can also print off the form from the website. Also by law, copies of the complaint forms and brochures are distributed to nursing homes and health care facilities, and must be placed and readily available for public use. The brochures offers information on how to contact the Attorney General, the State Long Term Care Ombudsman and other nursing home advocacy groups.
Upon submission, the complainant will receive an immediate electronic acknowledgement that the information has been received and the Complaint Investigation Unit (CIU) will follow up with an investigation. The BHS-CIU receives and processes consumer complaints against nursing homes, home health agencies, hospices, hospitals, freestanding surgical units, ambulatory surgical units, and end-stage renal dialysis centers throughout the State of Michigan.
This online complaint system makes it easier for advocates and loved ones to share concerns with CIU, and helps to strengthen the partnership in protecting the health and safety of our family and loved ones.
